Wedding Survey
Review this survey. You may find it helpful in planning
your event. If you request my piano
performance, you must copy this survey from the web site and return a completed
copy to me
(see Contract/Fees page for instructions to copy material
from a web page).
Live music for your wedding ceremony or reception is very important for your
memories of that day. I look forward to being a special part of your ceremony!
This questionnaire is intended to get you thinking about
items that can add to a smooth and very special ceremony. You may have thought
about some of the questions, and you may want to add comments to questions that
I do not list below. The important thing to remember: the day of your
special event is not the day to make changes that may be impossible to meet.
Let’s work together now, and avoid problems later!
List ceremony, reception, and rehearsal locations and
rehearsal dinner information.
Name and contact number for each venue.
If you are planning to have your event in Rocky Mountain
National Park, have you contacted their information desk at 970-586-1206 for
permission, permits or instructions?
Is there a keyboard on site? If yes, please describe.
Religious preference? If I need to “stay away” from certain
music, please tell me now!

Did you review and complete the enclosed suggestions for
ceremony schedule?
How many in the wedding party?
How many ceremony and then reception guests are expected?
Name and contact number of vocalist or other musicians
scheduled.
Name and contact number for wedding officiate.
Will a ceremony program be printed? If yes, send me a copy
no less than 10 days in advance of the event.
Vocalists, brass, harp, violin and string ensembles are
available. I can give you a quote for their performance. Yes or No.
If event is scheduled outdoors, is electricity available
for my electric piano?
If event is scheduled outdoors, is there an alternate site
reserved in case of bad weather? Does that site have a keyboard or electricity?
Please describe and give name and contact information.
List music selections that you may want to use as a
processional (entry of the bride) and recessional (bride, groom and wedding
party exit). I will review your choices with you and may suggest alternate or
additional selections.
If reception music is requested, do you want easy
listening, light classical, dance, jazz or easy listening style music? Do you
want a DJ for dance music? Piano music can be enjoyed during the dinner while a
DJ can make the reception celebration an enjoyable dance event in itself. Do you
need a quote for DJ service?
